Documentation for third party (disability, employer, attorney, etc…) is billed at hourly rate in 15 min increments

Missed appointment/cancelled less than 24 hr notice: hourly rate charged at provider discretion

 

Accepted Payments: check, credit card, venmo

We are out of network with insurance. If you would like, we will provide you with an itemized invoice which you can submit to your insurance company so that you may collect partial or full reimbursement depending on your insurance plan.
